INFORMED CONSENT
Coaching services through Window of Clarity do not include mental health services such as psychotherapy, counseling or substance abuse treatment. Coaching is not a replacement for mental health or substance abuse treatments. I agree that I understand the difference between coaching and mental health services. I also agree that I will find professional help for mental health issues if it becomes necessary.
After the first seven days from purchase date, Window of Clarity payments are not refundable. I agree to complete my Window of Clarity assignments each week, and to participate in my coaching phone calls as scheduled. I understand that if I don’t do these things, or if I need to reschedule a call without giving my coach at least 24-hour’s notice, I may be given the choice to leave the program or pay an additional $100.00 fee to continue.
