INFORMED CONSENT
Coaching services through Window of Clarity do not include mental health services such as psychotherapy, counseling or substance abuse treatment. Coaching is not a replacement for mental health or substance abuse treatments. I agree that I understand the difference between coaching and mental health services. I also agree that I will find professional help for mental health issues if it becomes necessary.
I understand that Window of Clarity payments are not refundable. I also agree to complete my Window of Clarity assignments each week, and to participate in my coaching phone calls as scheduled. If I don’t do these things, or if I need to reschedule a call without giving my coach at least 24-hour’s notice, I understand that I may be given the choice of leaving the program or paying an additional $100.00 fee to continue.
Window of Clarity is in addition to any legal work that the Reynolds Defense Firm may be doing on my behalf, and the money I’ve paid for the Window of Clarity is separate from any money paid or due for legal fees. Window of Clarity payments are not refundable and will not be deposited in the Reynolds Defense Firm trust account at any time. I agree to complete my Window of Clarity assignments each week, and to participate in my coaching phone calls as scheduled. I understand that if I don’t do these things I may be given the choice to leave the program or pay an additional $100.00 fee to continue.
